What Belongs at the Changing Table

• Fragrance-free wipes, or cotton wool and water. Fragrance and alcohol are the two most common irritants on a nappy area, and for a newborn plain water is best.

• A zinc-based barrier cream. The one product that genuinely prevents nappy rash.

• A changing mat that wipes clean.

• Spare clothes within reach — you will need them without warning.

The Changing Routine

• Everything within arm’s reach before you start. Never turn away or step away from a baby on a raised surface — falls from changing tables are one of the most common infant injuries there is. The floor is the safest place to change a baby.

• Clean front to back, always, and especially for girls.

• Clean inside the folds and creases, where soiling collects.

• Pat dry properly. A damp fold is where a rash starts.

• Barrier cream thickly — you should still see a layer. Do not rub it in.

• Wash your hands before and after.

What Not to Use

• Talcum powder. Inhaled powder is a genuine risk to small lungs, and it cakes in damp folds where it makes a rash worse. A barrier cream does the job better.

• Fragranced or alcohol-based wipes on irritated skin.

• Antiseptics and disinfectants on a baby’s nappy area.

• Steroid creams unless a doctor has prescribed one. The nappy acts as an occlusive dressing that greatly increases absorption, and on a fungal rash a steroid makes it considerably worse.

Are wipes safe for a newborn?

Fragrance-free, alcohol-free wipes generally are, though many guidelines suggest cotton wool and warm water for the first few weeks. If the skin is at all irritated, switch to water.

How much barrier cream should I use?

Enough that you can still see a white layer after applying. It is a barrier, not a moisturiser — most people use too little and rub it away.

When should a nappy rash see a doctor?

If it blisters, weeps, bleeds, spreads beyond the nappy area, comes with a fever, or shows bright red patches with small spots around the edges — the last suggests thrush, which needs an antifungal.
